Questionnaires, scales, interview guides and informed-consent forms. In these documents the structure is not formatting — it is the data.
In a questionnaire, an item whose number shifted or a scale whose direction flipped is not a formatting bug. It changes what gets collected, and it surfaces after the participants have already answered.
- Item numbering — item 7 stays item 7.
- Response options — the same number of them, in the same order.
- Scale direction — "strongly agree" stays at the end it was at.
- Reversed items — they stay reversed, and are not quietly "corrected" to the normal direction.
- Variable names — codes such as Q14_R stay exactly as they are, and are not translated.
- Scoring instructions — preserved with the instrument, formulas included.
Every one of the six is checkable in the file you received, and requires no trust. The terminology sheet and the bilingual view exist precisely for that check.
Before you upload — the licence
Many published questionnaires and scales are under licence, and the licence sometimes forbids third-party translation or requires the rights-holder's approval. Check this before you pay — we do not check it for you, and we have no way to know.

Readability level for participant-facing documents
An informed-consent form is read by someone who is not a professional, and that is exactly what an ethics committee will examine. You can ask for an adjusted readability level for such documents, without altering the content that has to stay exact.
The same nine files as any translation order. Two of them matter here in particular: the terminology sheet, which shows which professional term was rendered how, and the translation protocol — the document that explains to an ethics committee or a supervisor how the translation was produced.
